Our Services
We provide a variety of services to help a patient lose weight, enhance certain features of the face, reverse the appearance of aging and nutritional counseling. We will find the treatment that meets the needs of the patient during the consultation. Our weight loss services include HCG weight loss treatment, Infrared Body Wrap, Formostar Body Wrap, weight management, bariatric medicine, nutritional counseling and fat burning injections. We also offer cosmetic treatment options to enhance specific physical features of the face and body including Botox®, Juvéderm®, dermal fillers, face injections, lip injections, lipotropic injections, B12 injections, B6 injections, Voluma and more. The first step to begin treatment is to fill out a New Patient Information Form.
Patient Information
A New Patient Information Form will need the patient to write down their medical history, current condition, insurance, allergies and any other pertinent information we need to know about. For instance, if a patient has tried a specific weight loss treatment in the past that did not work, we need to know. With this information, we will be able to customize the treatment to the need of the patient. Our treatment will help the patient lose weight and offer instructions on how to continue living healthy after the initial treatment. We will also use this time to go over any other questions or concerns the patient has. Above all else, this type of treatment requires teamwork between us and the patient.
